PHYSICAL THERAPY
Physical Therapy focuses on helping patients relearn and improve their gross motor function. This includes activities such as sitting, standing, walking, rolling over in bed, stairs, and getting into and out of chairs. Our physical therapists address these needs by providing patients with individualized home exercise programs, patient and family education, prevention programs, targeted strengthening and balance activities, as well as consultation on appropriate assistive devices such as canes, walkers, and wheelchairs.

OCCUPATIONAL THERAPY
Occupational Therapy focuses on the patient’s ability to perform activities of daily living. This includes bathing, dressing, tying shoes, kitchen skills, and feeding. Our Occupational Therapists enhance or restore your function and independence with self care that may have been lost due to injury, disease, surgery, or aging. They are able to address these needs by providing patients with individualized home exercise programs, patient and family education, prevention programs, targeted strengthening and balance activities, as well as consultation on appropriate assistive devices such as reachers, splints, shoe horns, and adaptive kitchen utensils.

SPEECH THERAPY
Speech Therapy focuses on the diagnosis and treatment of communication, feeding, and swallowing disorders. Our Speech Language Pathologists work to develop comprehensive treatment sessions to best address your individualized needs. They are able to address these needs by implementing oral motor and sound production exercises, swallowing techniques, and assess for augmentative and alternative communication devices.

AQUATIC THERAPY
Aquatic Therapy is the use of water as a medium to provide patients the ability to perform their therapeutic exercises in a weightless environment. This form of therapy puts less pressure on joints by reducing gravity's effect, which can help improve flexibility and increase range of motion. It is a great alternative to land therapy, in addition to promoting cardiovascular fitness and general well being.
The pool, located at our Portage Park Clinic, is heated to a comfortable temperature and makes for a calming, alternative healing experience. Changing rooms are available for patients, and we make sure each patient feels comfortable by conducting all of our sessions in a private manner.

WORK CONDITIONING
The goal of Vital Rehabilitation's Work Conditioning Program is to help workers reach the point where they are able to perform the duties their job demands, and return to work.
Work Conditioning simulates the worker's actual job duties and helps them to redevelop job specific abilities. Each program is personalized to help the worker meet their individual goals. Workers are monitored by a licensed physical and occupational therapist to determine the day they'll be ready to return to work.
FUNCTIONAL CAPACITY EVALUATIONS
A Function Capacity Evaluation is an intensive, customized and objective evaluation of a worker's ability to safely perform and sustain physical work performance. In essence, it determines what the worker is physically able to do at any given point throughout the rehabilitation process.
At Vital, we use a variety of tests to determine what the worker can safely do when they return to work. Each test is administered and monitored by trained, licensed therapists, and uses computer technology to deliver accurate, reliable and consistent results.
A typical FCE will last approximately 5-8 hours, and can include tasks such as:

CANCELLATION POLICY
For every cancellation with less than 24 hours notice or no-shows, you will be charged a
$35.00 cancellation/no-show fee.
Cancellation fees must be paid prior to, or at the time of your next scheduled appointment.
Consistent Cancellations with notice:
If your therapy sessions are consistently being canceled due to any reason, your scheduled therapy may be canceled and you may be discharged from therapy.
Consistent cancellations are defined as missing 30% or more of your scheduled therapy sessions.
NO-SHOW POLICY
After a total of 3 no-show appointments or cancellation with less than 24 hour notice, your scheduled therapy time may be canceled and you may be discharged from therapy.
No-show appointments are defined as the therapist or office not receiving a call prior to the appointment and you not showing up for the scheduled appointment.
LATE ARRIVAL POLICY
If you are 10 minutes late for your appointment, you will still be seen but you will be charged a $15 late fee. If you arrive more than 20 minutes late, your appointment will be a cancellation and you will be charged a $35 cancellation fee.
